This document represents the formal agreement between the undersigned parent(s) or legal guardian and TLAP Sports Academy Preparatory (TLAP). By entering into this agreement, you acknowledge and fully understand the terms outlined herein, and you commit to adhering strictly to its provisions. The parties acknowledge that this AGREEMENT is binding and shall not be subject to negotiation or amendment due to typographical errors or misprints.

Terms of Enrollment:

Upon the submission of the deposit, it is mandatory to sign the enrollment contract. No refunds will be granted for the deposit or tuition payments once submitted. Your electronic signature holds the same legal weight as your printed signature, signifying your understanding and commitment to the enrollment process.

Enrollment Fees:

To secure your child's enrollment at TLAP Sports Academy Preparatory, a non-refundable $2,700 (SEC) must accompany the signed Enrollment Contract.

Tuition Payment Plan (2024 / 2025 Year Season):

Late Payments:

A late fee of $200 will be incurred for payments received after the 4th of each month, and it is essential that this supplementary amount be remitted simultaneously with the overdue tuition payment. In the event of a second instance of late payment, a subsequent late fee of $400 will be applied for payments made after the 4th day of each month. Should a third occurrence of late payment transpire, the parent will be contacted for a discussion on how to proceed with the outstanding payment. Additionally, the account will be subject to being locked and subsequently referred to the accounting department for further handling of remittances.

The agreed-upon tuition amount is to be paid through an (8) installment payment plan. This plan covers team gear, uniforms, (2) meals, and activity fees.

By affixing your electronic signature, you commit to paying the agreed-upon tuition amount according to this schedule. Failure to comply will result in dismissal from TLAP Sports Academy Preparatory. The terms of this agreement are governed by the laws of the State of Florida. In the event of non-payment, TLAP Sports Academy reserves the right to pursue legal action, including court proceedings, to recover all monies owed to TLAP Sports Academy Preparatory, plus associated lawyer fees. This pursuit in court will be executed in accordance with the laws of the State of Florida.

Withdrawal or Dismissal Policy:

TLAP Sports Academy maintains a stringent policy regarding the reduction or refunding of fees for absence, withdrawal, or dismissal. It is imperative for parents and students to thoroughly review and understand the outlined circumstances, as no reduction or refund of fees will be allowed under the following conditions:

Parents and students are strongly advised to carefully consider these conditions before proceeding with enrollment at TLAP Sports Academy Preparatory. This policy is in place to ensure transparency and adherence to the academy's commitment to providing a conducive and secure educational environment.

•Substance Use: The use or possession by the student of alcohol or any drug, narcotic, or an agent similarly classed, unless prescribed by and administered under the care of a legally qualified medical practitioner.

•Mental Health: Student encounters challenges in completing daily tasks due to mental health reasons, our primary concern is the well-being of the student.

•Endangerment: Actions by the student that may endanger other students, faculty, staff, or school property.

•Self-Inflicted Injuries: Intentionally self-inflicted injuries are not eligible for fee refunds.

•Boycotting: Boycotting of classes or any scheduled activity by the student.

•Force Majeure Events: Closure of the school or an inability to operate, whether partially or wholly, directly or indirectly, due to fire, flood, earthquake, elements of nature, acts of God, acts of war or terrorism, labor strikes, civil disorders, acts of governmental or judicial authorities, government-imposed shutdown and/or delay, pandemic, epidemic, public health care crises, quarantine, or other disease control measures.

•Facility Destruction: Destruction of any school facility due to any cause whatsoever.

•Operational Inability: Inability of the academy to operate and provide formal academic instruction, including closure for any reason.

•Military Service: Student being inducted or drafted into the Armed Forces, including alternative duty as a conscientious objector.

•Physical Altercations: Engagement in fighting on school premises.

•Non-Compliance and Disrespect: Not following academy rules and being disrespectful to staff.

•Withdrawal Circumstances: Early withdrawal or medical withdrawal. Additionally, no refund is available for withdrawal or absence from class attendance for the balance of the academic year due to early completion of academic requirements, early graduation, student’s temporary absence or suspension, or a change from resident to day status.

•Passing: In the unfortunate event of the passing of a loved one, we extend our deepest condolences to the affected family. We understand the challenges that come with such difficult times. However, it is important to note that tuition obligations, as outlined in the agreement, remain in effect.

TLAP Sports Academy Preparatory Florida Hotel Travel:

TLAP Sports Academy Preparatory places a high emphasis on the behavior and responsibility of our student athletes, even when they are off campus during travel. As part of our commitment to fostering a positive and respectful environment, students are expected to share rooms with 4 or 6 other athletes, depending on the hotel location.
We emphasize the importance of respecting hotel property and maintaining a disciplined and considerate demeanor. Vandalism or destruction of hotel property is strictly prohibited. In the unfortunate event that a student athlete violates this agreement, parents will be held responsible for any damages incurred, and applicable costs associated with the restoration of the property.

We appreciate the cooperation of parents and students in upholding the values and standards of TLAP Sports Academy Preparatory, both on and off-campus."

Confidentiality and Legal Disclaimer:

This agreement contains confidential and proprietary information. Unauthorized access or disclosure is strictly prohibited. Any unintended recipient must notify the sender immediately.

Agreement Terms for 2024 2025 Season:

This binding agreement defines the obligations between the PLAYER, PARENT, and TLAP Sports Academy Preparatory. It covers various aspects, including exclusivity, attendance, behavior, playing time, confidentiality, and more.

Financial Terms and Conditions:

The document outlines the financial commitments and the consequences of non-compliance. Payment terms are front-weighted to accommodate the academy's fixed expenses.

Legal Recourse for Non-Payment:

In the event of non-payment, TLAP Sports Academy reserves the right to pursue legal action, including court proceedings, to recover all monies owed plus associated lawyer fees.

Parent and Player Code of Conduct:

Both PARENT and PLAYER commit to abiding by a code of conduct, with specific expectations outlined for positive behavior, respect, and adherence to rules.

PLAYER:

I affirm that, as a player, I am committed to being a positive team member and demonstrating good sportsmanship consistently. Recognizing this as an extracurricular activity, I will maintain my grades and uphold appropriate behavior to ensure my continued participation. I will make every effort to attend and actively engage in all scheduled practices and events. In the event of inability to attend, I will communicate promptly with my coaches, providing reasons for my absence. I pledge to practice and play to the best of my ability, understanding that in team sports, the interests of the team supersede individual pursuits. I will refrain from criticizing teammates, coach decisions, referee calls, or opponents. At practices and games, I will conduct myself with respect, and I will never intentionally harm another player. Failure to adhere to these regulations may result in loss of game time or suspension from athletic participation.

PARENT:

I solemnly pledge to offer positive support, care, and encouragement for my child's involvement in TLAP Sports Academy Preparatory sports, abiding by this code of ethics. I will actively promote good sportsmanship by providing positive support to all players, coaches, and officials during games, practices, and other events. I prioritize my child's emotional and physical well-being over any personal desire for victory. I will ensure that my child plays in a safe and healthy environment, free from drugs, alcohol, and tobacco. I commit to refraining from using these substances in the playing facilities. Understanding that the game is for the athletes, not the adults, I will encourage my child to treat others with respect regardless of race, sex, creed, or ability. I will make a concerted effort for my child to attend all practices and games, promptly notifying the coach of any inability to attend. Timely payment of all requested fees before my child's participation is assured. I will refrain from discussing coaching staff decisions, training methods, player rotations, or disciplinary actions with other parents, players, or teams. Any violation of the parent/player contract will result in the immediate dismissal of my child from the program. I am committed to being a parent, not a coach, and will willingly contribute my fair share to any duties requested of me.

Drug Testing Policy:

The document details TLAP Sports Academy's drug testing policy, including random testing and consequences for positive results.

To ensure the safety and well-being of all athletes at TLAP Sports Academy Preparatory, participation in athletics requires signing this form. Failure to do so will result in the student-athlete being unable to engage in any organized team activities at TLAP Sports Academy Prep. If there are any questions, it is advisable to discuss them with the Head Coach.

TLAP Sports Academy Preparatory is committed to maintaining a drug-free and healthy athletic environment. As part of this commitment, student-athletes may be subject to drug and alcohol testing as outlined in the TLAP Sports Academy Preparatory sponsored athletic program, including adherence to the NCAA Banned Substances List available at www.ncaa.org. Testing will occur on an ongoing and random basis throughout the academic year, with all 14 student-athletes being selected for testing on several dates throughout each prep year (Aug, Sep, Oct, Nov, Dec, Jan, Feb, March).

Athletes may also be selected based on reasonable suspicion of drug use, and the testing panel may include substances such as synthetic marijuana, marijuana (THC), tobacco, cigarettes, vapes, e-cigarettes, Amphetamine, Barbiturates, Benzodiazepines, Cocaine, Ecstasy, Methadone, Morphine Oxycodone, Phencyclidine, and Tricyclic. The selection of student-athletes for testing will be both random and focused on those indicating reasonable suspicion of abuse.
Penalties for positive drug tests are in place to maintain a high standard of conduct:

First Positive Test:

•Meeting with AD and/or Sport Administrator and Head Coaching Staff
•Parent(s) or Guardian will ordinarily be notified
•Possible suspension from practice and/or competition
•Regular drug testing
•Mandatory counseling
•Subjected to further testing: A mandatory follow-up test will be required (the student-athlete's results must be negative)
•Test results will be placed in the student file

Coaches Use of Positive Test:

Student-athletes should be aware that positive test results may be disclosed to prospective college coaches, influencing their assessment of the individual's ability and potential roster placement. The head coaching staff may impose more severe sanctions than those provided in this policy.
Second Positive Test:

•Meeting with AD and/or Sport Administrator and Head Coach
•Parent(s) or Guardian will be notified
•Suspended from the team immediately/terminated
•Test results will be placed in the file and sent to college coaches currently recruiting the student-athlete

Safe Harbor:
Admitting to drug use and seeking treatment will not result in penalties. However, coming forward after being informed of an imminent test or a positive test result will not provide protection from this provision.

By signing below, I acknowledge and consent to the terms and consequences outlined in this drug and alcohol testing policy."

Consent:

By signing electronically, you consent to drug testing, accept the policy consequences, and allow disclosure of results for administrative and coaching purposes.

Acknowledgment:

The document concludes with an acknowledgment section, confirming the understanding and acceptance of the terms outlined. Original initials and signatures are required.

3.3. In the event of a student's withdrawal from the program after its commencement, regrettably, no refund will be issued.
